Dental implantation is the placement of a titanium root-like implant into the jaw bone followed by prosthetic restoration. It is the only method of tooth replacement that reproduces the structure of a natural tooth from root to crown: the implant absorbs chewing load and helps preserve bone tissue from resorption, which inevitably develops when a tooth is missing.

Dental implantation in Moscow today is a predictable and well-studied procedure, with survival rates of leading implant systems above 95% over a ten-year period according to systematic reviews.

Types and Methods of Implantation

Classic Two-Stage Implantation

The implant is placed into the bone and the wound is sutured. After 3–6 months, once osseointegration has occurred, a healing abutment is placed, followed by a temporary and then permanent crown. This method is optimal when bone volume is sufficient and anatomy is standard.

One-Stage Implantation

The implant is placed without an incision, using a flapless protocol, or with minimal intervention. This reduces the number of visits and total treatment time. It is used when bone density and volume are sufficient.

Immediate Loading: Implantation with Immediate Prosthetics

After implant placement, a temporary crown or prosthesis is fixed on the same day. This allows the patient to leave the clinic with teeth on the day of surgery. The method requires a precise protocol and sufficient primary implant stability.

All-on-4

All-on-4 is restoration of an entire jaw on 4 implants. Two straight and two tilted implants support a fixed full-arch prosthesis. It is used in complete or subtotal absence of teeth and may help avoid extensive bone grafting.

All-on-6

All-on-6 is restoration on 6 implants. More support points improve prosthesis stability and load distribution. It is used when bone volume is sufficient and higher rigidity of the restoration is required.

Additional Procedures

Additional procedures may include placement of a healing abutment, soft tissue grafting, increasing the volume of keratinized gingiva, and placement of titanium pins.

Indications for Dental Implantation

  • Absence of one or several teeth with sufficient bone volume.
  • Complete absence of teeth on one or both jaws: All-on-4 or All-on-6 protocols.
  • Unwillingness to use removable dentures.
  • Impossibility of a bridge due to absence of supporting teeth.
  • Tooth loss due to trauma, caries, or periodontitis.
  • Bone atrophy, managed in combination with bone grafting or sinus lift.

Treatment Stages

1. Diagnostics. CBCT, cone beam computed tomography, is used to evaluate bone volume and density, the position of the mandibular canal, and the maxillary sinus. Gnathological diagnostics is performed. If necessary, intraoral 3D scanning and digital implant planning are carried out.

2. Preparation. Professional oral hygiene and treatment of active inflammation, such as caries or periodontitis. If bone is deficient, bone grafting or sinus lift is performed with a healing period of 3–6 months before implant placement.

3. Implant placement. Surgery is performed under local anesthesia or sedation if preferred by the patient. Duration: one implant usually takes 30–60 minutes; several implants take 1.5–3 hours.

4. Osseointegration. Fusion of the implant with bone takes 2–6 months, depending on bone density, implant system, and loading protocol. A temporary prosthesis may be worn during this period.

5. Healing abutment placement. A minor procedure is performed to form the correct gum contour around the implant.

6. Prosthetic restoration. The prosthodontist fabricates and fixes the permanent crown or prosthesis. Material and construction are agreed with the patient.

Rehabilitation and Care

During the first 3–7 days after surgery, swelling, discomfort, and mild soreness may occur and are controlled with pain medication prescribed by the doctor. A soft diet is recommended for 10–14 days. Smoking significantly increases the risk of implant failure; the doctor discusses recommendations individually. Sports and heavy physical activity are restricted for 7–14 days.

Long-term care: the implant requires thorough daily hygiene, including an irrigator, special interdental brushes, or implant floss. Professional hygiene is recommended twice a year. With proper hygiene and in the absence of systemic diseases, modern implants can last 20 years or longer.

Contraindications

Absolute contraindications include oncological diseases during active treatment, severe osteoporosis, bisphosphonate therapy due to high risk of osteonecrosis, severe blood clotting disorders, and decompensated diabetes mellitus.

Relative contraindications include smoking, which increases the risk of implant failure, poor oral hygiene, active periodontitis, pregnancy, and adolescence before bone growth is complete.

The final decision on whether implantation is possible is made by the doctor after examination.

Frequently Asked Questions

Is implant placement painful?

The surgery is performed under local anesthesia. During the procedure, the patient does not feel pain, only pressure and vibration. After the anesthesia wears off, moderate discomfort may occur for 2–3 days and is controlled with prescribed pain medication. Intravenous sedation is available for patients with high anxiety.

How long does a dental implant last?

With proper hygiene and in the absence of systemic diseases, modern systems demonstrate survival rates above 95% after 10 years. A realistic service life is 20 years or more. The crown on the implant usually lasts 10–15 years before replacement.

Can an implant be placed immediately after tooth extraction?

Yes. This is called immediate implantation. It is possible when there is no acute inflammation and bone volume is sufficient. It shortens total treatment time. The doctor evaluates the possibility of immediate placement after diagnostics.

What if there is not enough bone for an implant?

Bone deficiency can be corrected with bone grafting or sinus lift, either before or simultaneously with implant placement. In some cases, tilted implants are used, as in the All-on-4 protocol, helping avoid extensive grafting. The amount of required preparation is determined using CBCT data.

Bridge or implant: which is better?

A bridge requires preparation of two adjacent healthy teeth and does not prevent bone atrophy beneath it. An implant does not affect neighboring teeth, preserves bone, and functions like a natural tooth. Long-term data show that an implant is a more predictable solution when one or two teeth are missing.
